Managing the Continuum of Care for BMT Patients (Panel Discussion)
Providing for optimal recovery and diligence in assessing for complications is paramount in the outpatient-inpatient continuum of care for BMT patients. The structure of communication and "hand-offs" are essential to safe patient care and management of complex treatment plans experienced by BMT patients.
This panel discussion will review how processes have been established to ensure safe patient care delivery across the continuum of care has been achieved within a community-based program, an academic-based program and a free-standing cancer center.
